FORM I-9 - U.S. Department of Justice, Immigration and
Naturalization Service


EMPLOYEE INFORMATION AND VERIFICATION

To be completed and signed by the Employee:

A.  Prospective employee completes Form I-9 in the employment
    center where the employee is being processed:

        1.   Office of Human Resources for staff employees at the
             Pittsburgh Campus

        2.   Departmental office at the Pittsburgh or regional
             campus of the college or school for all employees
             other than staff and College-Work Study students

        3.   College-Work Study Office at the Pittsburgh or
             regional campus for work study student employees

        4.   The Personnel Office in the appropriate medical
             facility for staff employees at MHCD and UHCP.

        5.   The appropriate Regional Campus Employment Center
             for regional campus employees.

B.  Prospective employee presents documentation as evidence of
    employment eligibility, and they attest to being either a
    citizen or national of the United States, an alien lawfully
    admitted for permanent residence, or an alien authorized by
    the Immigration and Naturalization Service to work in the
    United States.


EMPLOYER REVIEW AND VERIFICATION

To be completed and signed by the appropriate University
Responsibility Center or department:

C.  Examine verification documents presented by the prospective
    employee, insert the Document Identification Number and
    expiration date in the appropriate column (List A, B, or C)

D.  Certify the form I-9 by signing and dating in the blocks
    provided.


    Note:  A complete listing of verification documents
    acceptable for attesting to employment eligibility is
    provided in Exhibit C.
